I do not have a example of the guarantee letter, but a hand written letter, start with date, place where you life, dear consul or to whom it may concern, I would like to ask to provide a visa to my husband name, passport nr. Thanks in advance, name, ID nr, house address. It is the place from this website

she just addresses it to the consulate there and says her name blahblahblah, thai i/d number living at xxx address requests the consulate issue a Non-O year long visa to her husband, your name your passport number. and signs it.
No big deal, no special format

Copies of;
-frt&bk of the rose bordered marriage certificate
-frt&bk of your thai wife's i/d card
-thai wife's house book listing
-data page of your passport
-visa/entry stamp to Vietnam
You sign everything with your name on it and she signs everything with hers meaning you BOTH sign the marriage certificate copies
Those things the hand written letter from your wife asking the consulate to issue the visa along with a filled out application (that you can get there or download) AND 200US dollars gets you the visa.
Turn in your application one morning, pick up your passport the next afternoon.
