TLDR : Answer Summary
To cancel your ED Visa, you do not need a specific form. Instead, obtain a termination letter from your school indicating the end date. Then, visit the immigration office within two weeks before that date to have your extension cancelled.

nope, no form at all, you take a paper from your school saying you stop on xx date, you go to the immigration office up to 2 weeks before that date and they will cancel your extension and stamp you in until the date on the termination paper . ..

Do you have a visa, or an extension of stay based on being a student?

Just out of curiosity, is your visa connected to your daughter's visa? For example, if she's a minor, are you in Thailand as a parent to a child who is studying in Thailand?

no I’m connected with my husband’s retirement extension. She’s alone with that.
