Does any one have a form "Acknowledgement of overstay penalty"?
I have TM7 and STM2, but i can't find this document anywhere

PS the office in the trendy building takes nearly four weeks to get an appointment unless you pay 1650 baht for a walk in,The office is open monday and wednesday for three and half hours a day.

Just asked at cw about overstay as my new passport took nearly ten weeks to arrive resulting in 16 days overstay. The fine is five hundred baht a day up to and including the day you apply up to a maxium of

baht.If you go over 40 days you are asked to leave the country and upon return re apply. Which could be a major problem.Was told at the passport office that British passports are now taking twelve weeks to replace also the office in the trendy building is only open two days a week for three and a half hours,So if you need replacement passport go at least 3 months in advance.

They'll give you one when you apply for extension. Takes 2 minutes to fill it out and sign. It's been standard here in bkk since covid amnesty started and still using them since amnesty stopped.

I get what you are trying to do. I am an organized person myself and also would like to limit the amount of time needed to spend inside an immigration office. Yet, I gave up on trying to get all the forms necessary prepared upfront. Simply because, whenever and wherever I go, there were different forms needed or their design and details varied. Every office does things differently. Hell, even every officer. A functioning website with reliable, current details and documents needed - forget it. So in the end there is just one approach: Go there. Listen to whatever they want. Get the forms they give you. Source the ever changing supporting documents they request from you. And then go again and hope for the best. A yes, and don't get smart and call them first. In my experience, just because someone tells you on the phone what is needed does not mean is (still) true on the day you go there.

I had to do it in January at CW for retirement extension. It was put in front of me, I just had to sign it.
. I think it's a new initiative so if you do overstay they can wave a signed declaration under your nose, in case you plead ignorance. I knew nothing about it until the IO asked me to read it and sign it. All the order parts (name etc) she'd already completed. It was no big deal. Most people are aware that overstay is the worst crime any foreigner can commit in Thailand! 🤣
I’ve personally helped friends get their Covid extensions in Phuket and that form has not been needed (well since the last time they did it around March 30th).
I can’t comment on the retirement extension requirement because I’ve not personally done it or been there to help. But for the Covid extensions, three times now that form hasn’t been needed or signed. Perhaps different officers on different days have different requirements. Not an unusual occurrence here 😀
not sure which immigration office the op is applying to or if others require it, but I just noticed this one specifically mentions "Chiang Mai Immigration Office" on the top right of the form.
Well... he asked the same question in "Phuket Expats", so i guess Phuket, but maybe he just try to find the answer in any available group...
For the Form here... not sure it was labelled with Phuket or not, but i remember that the backside was a copy of a bank statement from a swiss guy... i was a bit jealous about the amount in his account... ;-)

What is that for? I've never heard of anyone being asked for it?
I was required to sign it almost every time, if not every time, I've been to immigration here in Chiang Mai in the last 4 months (extension, transfer visa to new passport, and change visa to non-o).
