Hi, anyone has a link to 60 Day Covid Extension Affidavit form(can't find it online)? Any other forms to be filled except that and TM7? I think i remember filling in three different forms in the past...Thanks!

P.S. I DO know that i will need affidavit, otherwise i wouldn't be asking around... Went to Bangkok immigration a few times already and every time had to fill it, just trying to avoid sitting there and filling in all the forms...
