Where can I find digital versions of the 'Acknowledgment of Penalties for a Visa Overstay' and 'Deposition of Foreigner Instance for Extension Retirement Stay' forms in Thailand?

Goodmorning everyone,

Last week, I went to the immigration office in Korat for the list of documents to be presented when requesting extension of stay for retirement. In addition to this list (same as previous years), I also received two extra forms:

1. The Acknowlegdment of Penalties for a Visa Overstay;

1. Deposition of Foreigner Instance for Extension Retirement Stay in the Kingdom.

Would anyone have a digital version of these 2 documents? I cannot find them on-line nor in the files of this group. I tried scanning them, but the quality of the copies I received is so bad.

Be aware at least one Immigration Office (Sukhothai) will not allow computer-filled paperwork. I have a dislocated shoulder with a broken humerus bone and they still won't allow any fully printed forms. My girlfriend has to WRITE it in for me and then I sign it.

Why don't you just fill out the form that was given to you. That's what I did even though it looks cruddy it was accepted when I did my extension of stay at the Korat office 3 weeks ago

I guess I will not have a choice, but I had to ask. I prefer to fill out my forms by computer.

I typed out the name of the form in thai (what was written on the top of it) and googled around but I can't find any form they've uploaded any where. Sorry about that..

I have made a pdf-file and uploaded it.

scan it into your pc and them convert it to a PDF and use the function to insert text.

looks really close to that one in the files section of either the 3 forms or the 4 forms that has the word "statement" on the top.

I don't think so. The deposition is about the bankaccount, which is not even mentioned in the statement.

the overstay form is in the files section in a PDF called 4 forms

It's the 3rd form in that group. The other ones are what chaengwattana gives people to sign.
