How can I pre-fill the form for the unable to travel visa extension in Thailand?

Is there a form file I could pre-fill for the "unable to travel" type of visa extension, please? I couldn't find it in the files page or via text search. Also, I wouldn't know what it's called or what it looks like.

You need an Embassy letter...each Embassy has their own...requesting a 30 days extension...if that is what you are asking about.

And its up to Thai Immigration whether to grant the extension or not

print it front to back on ONE piece of paper and if you don't fill it out online use BLUE ink when you do fill it out.
