TLDR : Answer Summary
For COVID visa extensions in Thailand, the TM7 form is required, which is the standard application for extension of stay. Additionally, if applying at Bangkok Immigration, four specific forms are required, with details on necessary documents and varying requirements depending on the immigration office used. Be sure to print the TM7 form double-sided, and check specific needs based on your local immigration office.

ALL extensions use the same "application for extension of stay form" the TM7
If you're printing it yourself print it on ONE piece of paper front to back π not two sheets.
I am in Mukdahan. And the forms they gave me for e.g. acknowledgement of overstay just have been one-sided. But no problem to bring both options to be prepared. π
You'll have to ask Mukahan if they accept TM7's from the internet, or if they allow single sided or double sided copies. Different immigration offices may have slightly different TM7's.
IF you are using another immigration office, you should use their forms they have at the front desk.
Different immigration offices have different requirements, but all require the following:
1. Your passport and:
a. a copy of your data ("picture") page
b. a copy of your most recent visa (if you have one)
c. a copy of your most recent stamp
d. a copy of your departure card
2. [Maybe required for Bangkok] A TM30, filed in the immigration office.
3. The forms (each immigration office may have different forms, and want to use their special forms)
4. [NOT required for Bangkok] A letter from your embassy saying you can not return, OR a print out from your embassy website saying they donβt issue the letters, OR a print out of a letter from the embassy saying they don't issue those letters.
5. 2x passport pictures
6. 1,900 THB.
Different immigration offices have different policies. *SOME* may require:
- The ID card of the property owner where you are staying
