I’m applying tourist (medical visa) for Thailand. They saids there that I need a copy of the medical/ health care registration and I didn’t get what that is??
Does anyone have any idea what that means? Like is it the referral letter ?

Visa and Certificate of Entry for Non-Thai nationals who need to receive medical treatment in Thailand (except for treatment for COVID-19), including their accompanying persons (no more than 3 accompanying immediate family members)
Step 1:
Applicant contacts hospital in Thailand that has been approved by the Ministry of Public Health (MOPH) to receive foreign patients and quarantine foreign patient and to send a request to the Department of Health Service Support (HSS), along with (1.1) Confirmation Letter (1.2) Affidavit of Support (1.3) Summary Form of Vehicles for Patients and other related documents such as a passport.
Step 2:
If approved, HSS will send the list of approved patients and accompanying family members to the Ministry of Foreign Affairs to inform the Embassy to issue a Certificate of Entry (COE) and visa.
Step 3:
Completed the Online Visa Application (OVA) at least 10 working days before the intended date of travel

You can't just come to Thailand on a medical visa. You must be in direct contact with the hospital you are going to be using, and depending on the procedure they might also require that the doctor is in direct contact and consultation with your regular doctor. Once everything you need done is verified and approved by the Thai doctor, they will start the process of getting your correct medical visa. You basically can't do it yourself.
you need a form from the doctor of exactly what is going to be done, and if you will be fit to travel after the procedure. A medical visa is not a set number of days. It basically expires, and you leave, once the doctor clears you to travel. You are not coming on vacation. You are coming for medical procedures
most medical procedures that a person comes to Thailand to have done, the hospital/doctor coordinates what is to be done, and the transfer of medical records from your doctor in your home country that you are seeing. As an example if you wanted to come over to have LASIK surgery done on your eyes, it would be scheduled in consultation with your doctor that recommended it.
