I am in the USA now. I had a bad rotator cuff tear that looks like it will need surgery and therapy. With no insurance I want to go straight to BKK.
I have already been in and out of Thailand 5 times within a year. So I don’t get turned around at immigration and can stay full length of treatment I would like to apply for medical visa.
Every Thai embassy website asks for 2 things.
1. appointment letter with treatment details
2. Hospital / medical facility registration certificate
Every hospital wants to offer me an appointment slip and a letter with treatment details. Every hospital is also confused by the meaning of #2 above. I understand #2 to mean the certificate they get from government agency to be a medical facility.
Does anyone know what I need to submit? I’m in pain and the visa application already takes 10 days. Trying to get this right the first time. Thanks in advance.🙏🙏

Ok, just spoke with embassy in new york. They are requesting the certificate to practice medicine from the ministry of health in thailand, that was issued to the hospital. They also said we can just copy their certificate from an online source. Thanks so much for everyones help. Im submitting what i have now and will update again.🙂
As you have ALREADY been in Thailand 5 times in the past year, it's possible that Thai Immigration Officers on arrival into Thailand may ask you Questions. Hopefully with an approved Thai Tourist Visa for approved surgery and medical care, you will be stamped into Thailand.
And once you have had the surgery and the doctors believe that you are fit to fly, it's doubtful that you can obtain further Medical care extensions of stay from local Thai Immigration offices.
The International Department of the Thai Hospital will be able to assist you with questions and obtaining the doctor's planned surgery documents and Medical care.
it's VERY New. There was a medical Visa before but during COVID-19 it was not issued. It's a 90 Day tourist visa for medical treatment MT. AND the steps to take to obtain it seem clear... Meet the requirements for and receive the MT Visa. As I understand it, it cannot be extended in Thailand once the medical care is completed. Doctors/hospital must submit medical reports/certificate requests to Thai Immigration for any application of extensions sought.
this is not what the e-visa website asks for now. It really looks like that was new requirements during Covid. Look at the date, and mention of quarantine…
