US: for the affidavit for your degree, what information needs to be on it?  They have a blank form but I'm not sure what information I need to include atm.  If anyone has an example (with no personal information on it) it would be super appreciated, I have my appointment for it tomorrow

Seeing as the US consulate doesn't "certify" degrees "officially" you need to fill out the blank affidavit and use it as a work around.

Mostly you put your name, DOB, passport number, received a degree in xxx from xxx university on xxx and it is real and accurate
