TLDR : Answer Summary
To provide verification of your income for your appointment with the US consulate, you can either type out a statement for verification or use a specific form. Some people share that an affidavit works since the embassy accepts what you state as long as you provide some form of proof. It is also suggested to prepare any necessary documents beforehand and download the required forms from the embassy website. It's advisable to check with the embassy directly for precise instructions.

I didn't bring any proof. It's an affidavit. The embassy is just saying you told them something and they believe it.

, don't use the income affidavit that's addressed to the immigration office and isn't the form you want.

Use the standard affidavit one and write what you need in the spaces.
