What does my friend need for a COVID extension at Phuket immigration on a tourist visa?

Hi has anyone got a covid extension at Phuket immigration recently? My friend is American and here on a tourist visa. He has already done his extensions, and wanting to now do a covid extension, so I am just wondering exactly what he needs at that office?
