What do I need for a 30-day visa extension at Phuket immigration?

I'm going for an extension (another 30 days) of my visa exempt entry in Phuket immigration office.

If you can't find him or her you can book into a hotel.

After arriving at a private address, your landlord, house owner or house master has to report your stay at Immigration Office within 24 hours. If not done you have to do this first before you can apply for the Extension of Stay. You need a copy of owner id card and copy blue housebook, if he don't want to joint you to make this report you also need a power of attorney signed by him/her and 10 Thb tax stamps. If he/she not understand, you can show him/her this movie:
