What documents do I need for my first Extension based on Marriage in Thailand, and where can I find the Married Confirmation Form?

Good Evening All !

I will be applying for my first Extension based on Marriage, Going through my paper works, If I would Miss anything some suggestion would be helpful :

1. Passport copy ( All used pages)

2. Departure Card copy

3. 2 copy passport size Photo

4. Thai Wife ID Copy (Signed)

5. Thai wife’s Tabian Baan Copy (Signed)

6. Married Certificate Copy (Signed)

7. Married Registration Copy (Signed)

8. Rental Agreement Copy

9. Apartment/Condo Thai ID Copy (signed)

10. Hand drawn map of place of stay

11. Photos with wife (Inside-outside of Apartment)

12. Bank Statement of 400k for 2 months

13. Letter from the Bank

** Where to get the “Married Confirmation Form”. Does anyone have a Sample photo of it. Appreciate your help.
