Aha, Korat. Normal is only passport. Did my first 90 days again after holiday trip. Had copies TM47, TM6 and copy entry stamp from passport. Lady trown all copies in the bin, looked at my passport, then computer, printed the paper with next date and ready. So think you need nothing as y'r passport.

I’ve never had to do that at Korat. No form, no copies. Just hand them the passport, 2 minutes later they hand it back with the new paper with your next date.
