What documents do I need for a 90-day NON-O visa based on retirement, including TM30 and photocopies of previous visas?

Hello, i need to get the 90-day non-o visa based on retirement, and i have two questions:

1- TM30: instead of filling up the required form, my condo gave me the attached document. is it sufficient? do i have to bring a copy of my contract?

2- in the list of needed documents shown at the immigration office, it says that i must bring a photocopy of 'all details of thailand visa'. does it mean my current visa exempt entry stamp+ 30days extension or... a photocopy of all the previous thai visas in my passport (many, including the retirement extensions i obtained before)?

okay, you're going to need photocopies of;

-the data page of your passport

-your 30 day arrival stamp

-your 30 day extension stamp

-your TM6 departure card

-your TM30 receipt

-probably your lease

-probably a map to your house

-the bank book, bank letter, etc to meet the financial requirements

a filled out TM87 application for visa (to go from the 30 day visa exempt to an in country 90 day non-imm visa) with a passport size photo stuck on it and 2000baht

remember you need at least 21 days left on your current stamp to apply for the in country Non-Imm visa at the Chiang Mai office
