TLDR : Answer Summary
To complete your first 90-day report at Phuket immigration, you will need to provide the following documents: 1. A completed 90-day report form (TM 47). 2. Your passport along with photocopies of the identification page, valid visa stamp, arrival/departure card (TM 6), and TM 30 form (proof of address). You can submit your report 15 days before or up to 7 days after the due date.
