What is the procedure after the first 90 days to report that you are staying in Thailand? Can this be done online, or do we need to visit the Immigration Office the first time?

If you're in a different part of the country than the one you entered, you can just go to the nearest IO, right? It doesn't need a special trip back to Phuket.
