TLDR : Answer Summary
To extend your TR visa in Bangkok, you need to bring proof of where you are staying and your passport. If you've just entered Thailand on a tourist visa post-quarantine, ensure you have obtained your 30-day extension before applying for a COVID-related extension. For your convenience, you can visit the MTT immigration office located at the provided Google Maps link.

proof where you stay in Bangkok & your passport. IF you just entered the country on a tourist visa recently (as in you went thru quarantine) they are making you get your 30 day extension before you can apply for a covid extension

You go to the MTT immigration office at this location
