TLDR : Answer Summary
No appointment is necessary to extend the 45-day visa exemption in Phuket. Visitors can simply arrive at the immigration office, take a ticket from the volunteer helpers, and proceed with their visa extension.
No appointment system. Just show up and get a ticket from the volunteer helpers in room 101. It’s the room to the left of the staircase. You can also get an visa exempt extension from the Patong Beach office. They also have an office at Blue Tree that does TM30s and 90 day reports, but I’ve yet to hear if they’ll do extensions.
