What do I need to do for a 30-day extension at Phuket immigration?

It's been a long time since I've had to visit phuket immigration for a 30 day extension. Can someone please give me a quick run down on how it works again? Do I need to pre fill forms prior to getting there? Passport photo prior to getting there? Can I wear shorts? Etc

Also I tried to look online about open dates around Christmas? Couldn't find. I'm looking to go around December 18

Christmas is not an official holiday in Thailand, but they close for New Year. If you are staying in a hotel it is easy but if you're staying in private accommodation you will need a TM30

Go in the morning with your passport, a passport photo and 1900 baht cash, complete the form and drop off your passport, the photo and the cash. Come back when they tell you to come back. I suggest you wear pants to be safe.
