For 30 days extension You need ONLY:

-passport

-copy of passport front page

-copy of stamp/visa page

-copy of TM6 form 

-filled TM7 form 

-two photos

You DON’T need to fill TM30/TM28

In Phuket they are not requiring TM30 confirmation for 30 day extensions. They do require it for long term extensions.

I'm guessing, and it's a guess please don't shoot me. That the thai address given on the OP's paperwork matches with that already provided by the hotel owner, so no questions. It may not be the case that they don't want tm30 info, they already have it.

Phuket wants to have a " prove " where you stay " normaly it's enough if you have the address and the name of your hotel written in the form ( Patong ) in phuket town they want a confirmation of the hotel , l
