For the STVs, different immigration offices seem to be having different requirements. The strictest immigration office for the extension of stay based on STV is Bangkok immigration. I am attaching an e-mail from Bangkok Immigration explaining what their requirements are for a 90 day extension of stay based on STV.
Regarding health insurance: You *WILL* need the same insurance policy that you used to get the STV. There are lots of reports of rejections for not meeting the health insurance requirements.
For housing: Bangkok Immigration require one of the following:
- Pre-payment for a hotel WITH the Owner's Tax ID number.
- A copy of your rental agreement
- A copy of your condo ownership book.
VERY few people have this visa. CONTACT your immigration office.
