Bangkok is strict, and requires the same documents that was used to submit the STV request. I'm attaching an e-mail I received from Bangkok Immigration about what is required for the 90 day extension of stay based on STV.

Thank you! Do you happen to know if it has to be the same medical insurance from a Thai company or if it can be an international one (that covers everything required)?
an insurance company that provides the dual insurance required. Cannot be just any travel insurance. Best you contact your current insurer for assistance
