Insurance FOR EXTENSION of the STV (Special Tourist Visa), you have to have Thai insurance for the durance of your extension. The initial insurance is not valid for extensions. Friend of mine had LUMA...

What Stuart said. I frequently go between Phuket and Pattaya, I will do my TM30 in one place and extend another. However, to extend you must have yout TM30 done there as well. So you can do your TM30 at the location where you wish to extend.
As far as insurance - I have no clue.

I’m not certain but would guess that an extension would require a filing of a TM30 address registration. The TM30 determines which office you use, as you use the closest to that.
In theory you might get away with staying in a hotel for a night , where they are required to file a TM30 for you. You ask for a screenshot of that filing and then go to the local immigration office there. No guarantee but you could try.
