TLDR : Answer Summary
To extend your visa in Chiang Mai for 1900 baht, you'll need to provide a passport-sized photo, a copy of the ID page and visa page from your passport, the admission stamp in your passport, a copy of your departure card, and complete an application form.

CM isn't crazy busy like Bangkok. Still get there early before opening to spend less time.

Form is called TM 7, you can down load from the files section of this page and print, it is one page printed front and back side. Or you get one at the Immigration Office.

No, they won't photocopy it for you, you have to get the photocopies and passport photos before you show up to the immigration office.

Yes, the page with your mug on it. :) Also your current visa, the entry stamp for your last entry into Thailand and the TM.6 departure card you received on your last entry.
