TLDR : Answer Summary
To apply for a 30-day visa extension in Thailand, you need the following paperwork: a copy of the data page of your passport, your entry stamp, your TM6 departure card, a completed TM7 application form (available at the immigration office), and a passport-sized photo. The fee for the extension is 1,900 baht. If you are in Pai, ensure you use an address within the service zone of the local immigration office.

You need a copy of the data page of your passport (that's the one with your picture on it) your entry stamp, your TM6 departure card, a filled out TM7 application for extension (which you can get at the immigration office) AND a passport sized photo. Those things and 1900baht get you a 30 day extension :P
