In Koh Phangan there was an additional form I had to sign but they give it to you (maybe TM7, but not sure)

In some Immigration Offices there can be two additional STM forms to sign, one is regarding overstay acknowledgement.

You need to get a TM30 form from your hotel or accomodation and your passport and 1900 baht I think you need 1 passport size photo i can't remember

Make your application for the 30 ADDED days extension with your local Thai Immigration office. Be sure you have a TM30 address registration on file with Thai Immigration.

Most Thai Immigration Offices accept the application within 30 days from your ending date...

BUT Jomtien Pattaya Thai Immigration office will only accept an application during the week before your ending date.
