You need to get a TM30 form from your hotel or accomodation and your passport and 1900 baht I think you need 1 passport size photo i can't remember

THESE are the common found requirements for the 30 days extension.
The 30 days are ADDED to your current ending date.
Most Thai Immigration Offices accept the application within 30 days from your ending date...
BUT Jomtien Pattaya Thai Immigration office will only accept an application during the week before your ending date.
