What are the requirements and recommendations for applying for a Destination Thailand Visa (DTV) for medical treatment in Thailand?

Dear all,

I need to apply for a DTV (Destination Thailand Visa) for medical treatment in Thailand and I kindly ask you to help me clarify some doubts, so as to avoid my application being rejected:

1)"Applicant should not apply for visa more than 3 months before the date of intended arrival."

The regulation says that I cannot apply more than 3 months before the date of intended arrival in Thailand. This means that, if I want to leave in August 2025, I cannot apply yet, right? If today is March 8, 2025, I can apply for a visa with a maximum departure date of June 8, 2025, correct?

2)"Financial evidence: amount of no less than 500,000 THB, e.g. bank statements, sponsorship letter."

Is a letter from the bank stating that my balance is more than 500,000 THB okay?

3)"Document indicating current location."

Is a proof of residence with my Italian address sufficient? Or do I also need a copy of my ID card?

4)"Proof of confirmation to attend the activity or letter of appointment from hospital/medical center."

Is a medical certificate from the hospital stating that I need to undergo medical checks for at least six months valid as documentation?

5) Hotel/flight reservation.

In the DTV visa application I do not need to attach any proof of hotel or flight reservation to Thailand, right? Unlike the 60-day tourist visa, for the DTV you only need the documents listed above, correct?

1/ yes you shouldn't apply more than 3 months before your intended flight date. However, having said that you could say your intend to fly in May (for example). Find flight number for said date and then once approved they don't care when you choose to enter Thailand people's plans change all the time. The visa is for 5 years so come whenever you want.

2/ you need bank statements. More and more embassies are asking for 6 months of bank statements. The most recent one needs to be at least 500k

3/ proof of current location can be your driver licence

5/ no need to upload flight details. For accommodation a lot of people are booking fully refundable thai hotels on agoda etc. Then cancelling it after being approved

The embassies have said many times to not apply unless intended travel date is 3 months from application date.

1) the visa is good for 5 years so apply whenever you want

2) your actual bank statements

3) something like a utility bill in your name, driving licence or similar

4) you should get a 6 month treatment plan signed off by a recognised doctor and stamped by the hospital

5) you can just supply the flight numbers without actually booking
