I have some questions regarding the application for a re-entry permit (TM.8 form). Any help would be greatly appreciated.
1) Where it says "My previous visa for Thailand, etc", do you always use the first and original visa issued (the laminated one? ours is a "non-O" issued in Penang in June 2017)
2) Where it says, "I arrived in Thailand, etc", do you always use the last date you arrived at an immigration counter? (in our case the date is July 15th when we returned from an overseas trip to CNX)
3) And where it says "And have been permitted to stay up until, etc); What if you've since received a new extension and therefore the date doesn't match the last "admitted until stamp" from the last time you were admitted? (ours is based on retirement and Do you use your latest "permitted to stay until" date?
4) Are you supposed to staple or tape the 4 x 6 cm photo to the TM8 form?
