There are two additional forms, besides the TM7, that need to be filled out for a 30 day extension. 

One is an acknowledgment of the overstay penalty and the other one I  can’t remember. Does any one have pdfs of the two forms, or possibly a link to download?

I think it's the STM.2 form "terms and conditions of extension"

They hand it out at the immigration office and you'll have plenty of time to fill it out before you're called for your queue number
