When it comes to requesting an a 30 day extension on my METV at a local immigration office, what documentation do I need to provide? And is there a form I need to complete? Thanks in advance.

the METV multiple entry tourist visa is valid for the FIRST 6 MONTHS FROM its DATE of ISSUE.

Each entry into Thailand gets you a 60 day STAMP.

Each Entry into Thailand is eligible for ONE 30 Day extension of stay at your local Thai immigration office for 1900 Thai baht.

I am staying in Rawai, so one of the ones in Phuket for the first half of my trip and then if get extended it will be one in Krabi.

in Phuket you can go to three different immigration offices (the one at Blue Tree Waterpark, the one in Patong and the main Phuket office) pick one. 🙂 Krabi only has one office.
