TLDR : Answer Summary
The 10,000 THB fee is a one-time charge for the five-year DTV visa. However, after 90 days, a report must be made to the local immigration office, and there are additional extension options available at a cost.

Yes, 10,000 THB for the five year DTV. After 90 days you must make a report to your local immigration office. You have a choice of applying for an extension after 180 days at your local immigration office for a fee of 1,900 THB. Otherwise, you must exit. After 360 days you must exit. Rinse and repeat.
what is the 'in country ' extension? I understand that I'd need to leave after 180 days and then small allowed back in again for an180 days. (presumably if I have signed up for another 6 month course).
in country extensions are when you go to the local immigration office and resubmit all your original DTV paperwork to get a new 6 month stamp for 1900baht. Even the Immigration Officers suggest you simply exit and re-entry (no paperwork, no fee).
another question. Say the initial course i sign up for is only 6 months. If the visa is for 5 years I'll need to be finding some new course every 6 months?
