Thai insurance is never compulsory for a visa from the UK.
A personal friend obtained an OA from London with Nationwide travel insurance 2 years ago when they were at their strictest. I used the same to get a 90 day O visa.

It depends. If you're getting a visa that requires health insurance you'll have to buy Thai. If not, you may find better value getting at home. For Thai based insurance misterprakan.com is a broker for all the main issuers
