TLDR : Answer Summary
The user initially inquired about the necessity of a medical certificate upon arrival in Thailand. Several comments clarified that the medical certificate is currently only needed for the STV and OA visas. Had my Doc complete this (even back 12 yrs ago) totally outdated certificate. He thought it was hilarous. Surely you can find a MD to complete it.

it’s still the current form and yes so old! It is needed for an A-O visa from Oz and a few years ago I had a similar problem as no gp would sigh it without expensive and time consuming tests! Luckily my specialist did! I had been living in Thailand fir years so didn’t have a Oz gp anymore

My doctor signed mine, obviously he knows my health very well so he didn’t hesitate

It's only STV and OA which require it. Have you tried your GP? I took mine years ago to my GP and knowing my medical history, he signed it without batting an eyelid. He said some of the tests would cost hundreds of dollars and didn't see the point as I would have exhibited some symptoms.
