Do I need go do something on this website https://www.boi.go.th/ I also noticed medical certificate required saying I'm free of diseases. Is there a template for this and can the local GP sign it?

Download the application forms from your country thai embassy or consult the medical form is there. If you have a good relationship with your gp and they know your medical history they will sign it easy I didn't do any tests. OA and OX in Australia at least have nothing to do with BOI

The AO visa doesn’t need Board of Investment endorsement (that I’m aware of from any overseas embassy but please link details if you find that’s your case).

Getting a local GP to sign off on the medical part can be hard as they don’t generally do the specific tests required. Have a chat to a local (to Thailand GP) here.
