What Medical Certificate is required for the O-A visa application for UK citizens in Thailand?

UK citizens, my O-A visa is on hold as the Medical Certificate I sent (which my GP said would be ok 😮) is not what the embassy requires?
Has anyone in the UK, got an example or even better a pdf of the correct certification?
It’s this
Ministerial Regulation No .14 (B.E.2535) which include Leprosy, Tuberculosis, Elephantiasis, Drug Addiction, third stage of Syphilis

You could try asking Dr. Donna. She does the fit2fly certs over facetime for 400 baht, I don't see why this one would be any different. You can ask her on FB or through her website. *******************************
