How Can I Extend My 30-Day Visa Exempt Stamp in Thailand?

Used search function and google... still at a loose end. If one want's to extend a 30 day visa exempt stamp by another 30 days, is there a downloadable form that can be filled out prior to going to CW or must the form be completed upon arrival? Also, any new requirements to be aware of? Thank you in advance.

yes the new requirements are: 1 Application form filled in and signed by you, 1 passport picture, 1 copy of the ID page of your passport, 1 copy of the Admitted until stamp inside your passport, 1 copy of the TM6 departure card, 1.900 Thb for the fees and your passport so they can put in a new stamp.
