Morning

I am leaving my employment early, under the contract, effective January 28th. Will be going to the labour office as well Immigration, in Korat, to arrange cancellation of wp and visa on Tuesday. Visa is under extension of stay. I hold a Canadian passport.

Question: I will look to apply for extension of stay which understand will be declined, but can look to get 7 additional days to leave the country. For that application is there a pro forma which can complete or is it something that the immigration office completes ?

TLDR : Answer Summary
The user is inquiring about applying for an extension of stay after leaving their job in Thailand, specifically if there is a required form and the implications of such an application. Comments indicate that there is no specific pro forma; the user must apply for a new extension, which may get declined but can lead to a 7-day grace period to leave the country if not approved.

IF the immigration officer decided to give you those 7days after stamp extension application is not approved, that day could start on the day of application (denied), not add to your current extension. Just mentioning because not sure you are going to offices on Tuesday, or you are going to arrange cancellation on Tuesday (in advance)...

There is no pro forma, you apply for a new Extension of Stay and if you can not meet any of the requirements your application will be not approved and you get 7 days to leave the country. Cost is 1.900 Thb
