I dont think he need the letter to cut off the medical visa.

Suggest to ask at immigration directly.

I think the required documents are the same with Phuket.

1. Passport

2. Passport photo

3. TM7 form

4. Sor Tor Mor 2 Form

5. Covid Form (To specifythe reason why you need to extend visa)

6. Copy of passport, latest visa, departure date stamp, TM6, TM30

my friend help me. We went to the company where he work and help me. Still waiting for result☺️

You should not need a letter to cancel the extension of stay based on medical purposes. If he is still being treated, he can get another extension of stay based on medical reasons.
