Does the address registered with TDAC need to match the address provided for my 30-day extension? The immigration office requires the house master to complete a notification of residence form for foreign nationals, but I have recently moved to a relative's house.

No, and since you already exited out of Thailand and will make a new entry into Thailand again, a new TM.30 should be filed regardless, as this is a new entry into Thailand and the new TM.30 would reflect where you are staying now

Then your relative should complete the TM30 and give you a copy of the receipt which you take with you when applying for the extension

No it doesn’t, as you have moved house the owner of the property is required to lodge the TM30 within 24 hours of your arrival at the property.
