I’ve done several there (Samui) over the years and they’ve never checked. Just TM7 and the required fee etc. Doesn’t mean they won’t though.

Tell your friend to fill in the TM7 and pay the 1900 baht. Can’t see how they’d suddenly introduce a hotel signature, lots of people tour the islands here and extend in Samui - it would cause chaos with all the backpackers if they did. Good luck 😉
I do not believe Chiang Mai has ever needed a TM30 for a tourist extension. They definitely do need it for longer-term visas but not for tourist extensions. I have talked to people who extended recently in Chiang Mai and confirmed they did not need TM30 receipts. One thought they did but I enquired further and they were confusing it with the TM6 departure card, which CM immigration does want.
People mix up the different visa types, the requirements and other issues (like the queuing) are not the same in Chiang Mai between tourist and other visas.

I think Bim Smith is confusing the TM30 document with the TM7 document needed when applying for a 30 day extension to a 30 day visa exempt.. if so it’s turned a very simple question into a complicated mess for the OP. 30 day extension to a 30 day visa exempt = a visit to Maenam immigration office, fill in a TM7, take a photo, your passport, TM6 departure stub, 1900 baht and wait for about an hour.

They did in Phuket, Pattaya, Chang Mai, Immigration seems to not care if it create chaos for a while. But as Bim Smith not answered yet, I can not say you need or not. Best is to take a TM30 form, copy ID landlord and house book both signed by him/her if you stay on private address or hotel receipt.
What kind of document are you talking about, the TM7 form is an application of Extension of Stay and must be signed by the applicant, but I read here, a hotel or your wife

As more immigration offices are starting to enforce the TM30 rule, I will not be surpised that Samui is one of them, specially after a swat team went to a house and the foreigner was not living there for years. Big joke was not happy.

I'm not on Samui and the answers given are not clear to me, that is why I asked the question: Is Samui Immigration asking for the Notification of Stay, registration of address done with a TM30 form, because I do not believe that Immigration wants a hotel to sign your application for an Extension of Stay.
I am not on Samui i am asking for a friend and they are on a island outside Samui hence going with a ferry 2 hour back and forward IF they would ask for the form

I not understand this, You got it from the section and you do not like to use a ferry. What did you get from the which section and no other options to visit Island using a ferry, it was your choice to live or stay at an island, not Immigration.

Ok, the OP’s was asking if he needed the hotel to sign an application for a 30 day extension on his visa exempt. I’ve posted the document that was sourced from Samui immigration last week, no mention of it being signed by the hotel. I myself have never had to do that on Samui, nor has anyone I know. Samuel Fila also lives on Koh Samui and is familiar with this process. So again, unless it’s been changed in the last week or so I guess the answer is no, you do need it signed. However if Anders wishes to play it super safe I could send him the pdf version of it if he wishes to get it signed in advance.

the hotel had to sign the extension form and were given a different form also to sign. Our immigration has never asked for it before either which is why we didn't think it was needed.
I agree that everything should be done by the letter, just never heard of it been asked for in Koh Samui. It’s not a requisite on the form I posted either.
they just only weeks ago. Had to drive back to the hotel taking 2 hours to get it signed

That’s the current document from Koh Samui immigration in Maenam. Unless there’s been changes in the last week or so they do not ask for a signature from the hotel.

Passport photos, copies of Passport, arrival card and entry stamp. They can do it there for you at a cost.
