TLDR : Answer Summary
The arrival card is generally required for the 90-day reporting process (TM47), as it's a mandatory field on the form. If you lost your arrival card (TM6), the immigration office may provide a replacement or require a police report before proceeding with your 90-day report.

Totally up to the immigration office.

As a rule YES you need it seeing as it's a required field on the 90 day report form (TM47) AND on the online 90 day program.

Best case is they'll give you a new one, worst case is they'll send you to a police station to get a police report BEFORE they'll give you a new TM6 departure card and let you do a 90 day report.

Guess you'll ind out when you get there
