I think it's important that there is a letter in the old passport, from the embassy starting the number of old and new passport, then they can do the transfer, to show that they are connected. I did some months ago.

No problem at All... Just take both they know how to do.. The embassy or consulate will give you a letter for the new one
Just take your old and new passport to immigration and they'll sort it out. There is No fee to transfer it. Your embassy should provide you a letter or stamp in new passport requesting transfer. They wont process a yearly extension on a passport that's about to expire.
