TLDR : Answer Summary
The notice in question is related to a 90-day report (TM47), not a TM30. You need to complete the TM47 report before April 25th. It's suggested to file both the 90-day report and your visa extension application on the same day. You can file the TM47 between 15 days before and 7 days after the due date without incurring a fine.

That notice (the little paper in your passport) has NOTHING to do with a TM30 (that's filed when you move IN to a place).

This notice is that you need to do a 90 day report using form TM47.

and if you're going to apply for a covid extension you need the same documents you needed last time to get one. It's a NEW extension you're applying for

nothing but a filled out 90 day report form. You already got an extension of some type or you wouldn't have that form stapled in

90 day reports are very easy in Chiang Mai and take 5 minutes. Just follow the indications for the drive in and give your passport to the officer at the window.

nope you do ALL that at the main immigration office at the airport

The Promenada is only for people to do 90 day reports that have been doing them there (in Chiang Mai) before
