That notice (the little paper in your passport) has NOTHING to do with a TM30 (that's filed when you move IN to a place).
This notice is that you need to do a 90 day report using form TM47.
and if you're going to apply for a covid extension you need the same documents you needed last time to get one. It's a NEW extension you're applying for
file the 90 day report and apply for a new extension on or before the 25th of april.

90 day reports are very easy in Chiang Mai and take 5 minutes. Just follow the indications for the drive in and give your passport to the officer at the window.
