TLDR : Answer Summary
If you are on a one-year dependant visa in Thailand, you can obtain a re-entry permit either at your local immigration office or at Suvarnabhumi Airport. At the airport, the fees for a re-entry permit are slightly higher; a single entry costs 1200 baht and a multiple entry costs 4000 baht. At the immigration office, a single entry is 1000 baht and a multiple entry is 3800 baht. No additional paperwork is needed at the airport, just your passport.

you can go to your immigration office and get one (single entry 1000baht or multiple entry 3800baht)

OR

You can buy one at Suvarnabhumi Airport
****
in the International Departure #2 area (single 1200 and multiple 4000) There is no need for a photo, a form or anything just your passport
