TLDR : Answer Summary
Yes, you can obtain a re-entry permit from the immigration desk located at Chiang Mai Airport before entering the immigration counters. You will need to fill out the T.M. 8 form and can have your photo taken there for a fee, if you do not have one prepared. Be aware that the area can become crowded, and you might face long lines.

Both me and my father has used that desk to get re-entry permit. They have T.M. 8 form, and will take your photo of you don't have one (taking photo is not free).

Do note that, because of the increase of the travelers, the security checkpoint AND the carry-on baggage check point has move into the area that's in front of the Black Canyon Coffee.

And yes, line can be very long.

You can indeed get a re-entry permit from the Chiang Mai Airport IF you are flying out internationally.

I found this online:

The desk is up the escalator from the international check in counters, U-turn through security and before you get to the immigration counters, on the left. If nobody is staffing the desk, the immigration officers will come over from the exit gates.
