TLDR : Answer Summary
You can extend a TR 60 day visa at the Ratchaburi office if that's where your TM30 is filed. No appointment is needed based on available information. You can apply for an extension due to COVID-19 without needing to first get a 30-day extension. Required documents include your passport and copies, a filed TM30, specific forms from the office, a letter (or proof of no letter) from your embassy, two passport photos, and a fee of 1,900 THB. Be aware that requirements may vary at different immigration offices.

You're out of nothing applying for a 60 day extension of stay based on unable to travel due to COVID-19. The worse that would happen is they would give you a 30 day extension of stay based on tourism instead.
REMEMBER at most immigration offices, there is an under consideration stamp.
Different immigration offices have different requirements, and I don't know what Ratchaburi's specific requirements are, but all immigration offices require the following:
1. Your passport and:
a. a copy of your data ("picture") page
b. a copy of your most recent visa (if you have one)
c. a copy of your most recent stamp
d. a copy of your departure card
2. A TM30, filed in the immigration office.
3. The forms (each immigration office may have different forms, and want to use their special forms)
4. A letter from your embassy saying you can not return, OR a print out from your embassy website saying they don’t issue the letters, OR a print out of a letter from the embassy saying they don't issue those letters.
5. 2x passport pictures
6. 1,900 THB.
Different immigration offices have different policies. *SOME* may require:
- The ID card of the property owner where you are staying
The British Embassy no longer issues visa extension letters.
On Monday 2nd November 2020, the British Embassy Bangkok received verbal confirmation from Thai Immigration Bureau that Embassy letters are not required to extend visas within Thailand. Instead, you can complete an affidavit at your local Immigration Office.
If you want to remain in Thailand after your current visa expires, or you have questions about your immigration status in Thailand you should speak to your local Immigration Office or visit the Thai Immigration website.
Yes it will be required to get the 60 day extension of stay based on unable to travel due to COVID-19. If you are using Bangkok Immigration, the affidavit can be found out
