I think you would have to have 2 bank accounts with 800 k in each . 2 non O Visas and 2 Extensions of Stays

Hospitals have excellent neurology centers. I get lamotrigine for epilepsy and never had a seizure in Thailand. I git a confirmation letter from bumrungrad Hospital that i used to apply for DTV medial treatment

Yea DTV on his epilepsy ..but as Alain said..need confirmation letter ..I had one and went through smoothly ..on Dental plan

Not on your visa. He needs an own one. If you wait until he is 50 it will be much easier as he can apply also for an “retirement” visa.

I'm retired in Thailand and have a pre-existing condition M.S and insurance is impossible with that. Have you looked at managing the epilepsy here? Is he on medications?
