To any Canadians: I have just been, in person, to the Canadian Consulate in Chiang Mai. They are able to provide Affidavits for you to use with Thai Immigration in an attempt to get an extension on your Visa (which in my case is a Tourist Visa). I have NOT yet been to Thai Immigration to verify if this Affidavit letter is sufficient yet, though I will be doing so early next week and I will post here again with the results. Below is my (redacted) Affidavit. Note that YOU will need to fill out and write the Affidavit content. I strongly suggest indicating that you are "Unable to travel due to the COVID-19 outbreak" and are "requesting a special circumstance extension of your Visa" from the Thai Immigration Bureau. It is also likely in your best interest to include your current address.
TLDR : Answer Summary
A Canadian visitor shares their experience obtaining an Affidavit from the Canadian Consulate in Chiang Mai, which may help in requesting a visa extension due to COVID-19. They plan to verify its sufficiency with Thai Immigration and encourage others to include relevant details in their affidavits.
